Drinking Water System Name/Address Changes
The drinking water system owner or the owner's designee should contact the local DEQ Regional Office or District Health Department drinking water staff to report any changes to the name of the system, the name of the legal owner of the system, the owner's contact information or changes in either the mailing or physical address.  Go to Contacts to find out who your local drinking water staff contact is.
 
Wastewater System Name/Address Changes

The wastewater system owner or the owner's designee should report any changes to the name of the wastewater system, the name of the legal owner of the wastewater system, the owner's contact information or changes to either the mailing or physical address.  To report the changes, go to Owner Information Form and completely fill out the form and submit to:

The wastewater system owner is required to report a name change of a Responsible Charge Operator or Substitute Responsible Charge Operator to DEQ within 10 days following the change in personnel.  To report changes in either a responsible charge operator or a substitute responsible charge operator go to Public Wastewater System Operator Licensure Record Form

Drinking water and wastewater operators are individually responsible for keeping their personal contact information up-to-date.  For any changes to your name, address or contact information go to the Idaho Bureau of Occupational Licenses (IBOL) Web site at Name/Address Change.
